Replace Your Adult Limited-Term Commercial Driver License (CDL)

  1. What You Need to Know

    When applying for a replacement commercial driver license, the expiration date of the new license will be the same expiration date as your current driver license.  However, if your driver license expires in the next six (6) months, you would be eligible to renew your driver license.

    LOST OR STOLEN
    By completing the duplicate/replacement license process your previous license will be canceled.

    NEW ADDRESS
    You are required to change your address on your driver license no later than 30 days after moving.  You will not be able to change the address online as Federal regulations require that your current address be printed on your CDL.  Additionally, you will be required to provide proof of current Utah residency.

    NAME CHANGE
    To change your name on your driver license, legal documentation authorizing the name change is required.  These include a certified state marriage certificate, divorce decree, adoption, citizenship, guardianship, or court documents indicating the name change.

    Attention: By law, you may not possess more than one REAL ID-compliant certificate at a time (e.g., a Utah license and a Colorado ID.)
  2. What You Need to Do

    • Schedule an appointment to visit a CDL driver license office.
    • When you arrive for your scheduled appointment, you will take a photo.
    • After your photo, you will have a seat; when your number is called, you will present your documents to the examiner, take an eye test, and pay the fee of $23.00.
  3. What You Need to Bring to the Office

    • Bring your current Utah limited-term driver license if you have it or something with your name on it.
    • Bring your current lawful presence identity document.

    • Bring two (2) documents providing proof of your Utah address if you have moved from the last known address on your Utah commercial driver license.  Federal regulations require that your current address be printed on your limited-term commercial driver license.
    • For a name change, bring the authorized documents listed above.
  4. When Do I Get My License?

    After all requirements for a limited-term CDL have been completed, you will receive a temporary limited-term CDL with a photo on the same day.  The completed limited-term will have an expiration date of your legal presence document or five (5) years, whichever is sooner; or 1-year from the date of issuance if there is no expiration date, and will be mailed to you in approximately 4-6 weeks.
